The Big Three Filters: How to Narrow Your Search
Before you look at a single plan name or premium price, you need to run your situation through what we call "The Big Three Filters." These filters determine whether you should be looking at the ACA Marketplace or private alternatives like Short-Term Medical (STM).
1. Tax History (The Subsidy Filter)
The ACA is built on tax credits. To get a plan that costs you $0 or a very low monthly premium, the government looks at your projected household income. If you have a clean tax history and your income falls within specific ranges, the ACA is almost always your best bet. However, if your income is hard to track or if you are hitting the "Subsidy Cliff," your strategy has to change.
2. Pre-existing Conditions
This is the "hard" filter. If you have a chronic illness, are planning to grow your family, or have a surgery on the horizon, the ACA is your only real choice. ACA plans are legally required to cover pre-existing conditions. Private plans, like STM, use medical underwriting. This means they can, and will, deny coverage for conditions you already have.
3. The Subsidy Cliff
This is where many of our middle-to-high-income clients get stuck. The "Subsidy Cliff" is the point where you earn too much to qualify for any government help. Currently, for an individual earning over roughly $64,000 or a family of four earning over $132,000 (approx. 400% of the Federal Poverty Level), those tax credits disappear. At that point, a Bronze ACA plan might cost you $500+ a month for a very high deductible. This is when we start looking at other PPO options that might offer better value for your dollar.

Deep Dive: Texas, Florida, and Georgia
While all three states use the federal marketplace for ACA plans, the rules for private alternatives and the "budget" carriers vary significantly.

Texas: The Land of Flexibility
Texas is unique because it has fought hard to keep private options like Short-Term Medical flexible. While federal rules often try to cap these plans at four months, Texas state law allows for much longer durations, up to 36 months in some cases.
For a healthy 1099 contractor in Houston who doesn't qualify for a subsidy, a 12-month STM plan can act as a high-quality nationwide PPO for a fraction of the cost of an unsubsidized ACA plan. However, you have to remember that STM is not Major Medical. It’s a bridge, even if that bridge is a long one.
Florida: The Competitive Marketplace
Florida has one of the highest ACA enrollment rates in the country. Because so many people use the marketplace here, carriers like Ambetter and Molina are incredibly competitive. If you are looking for affordable health insurance in Florida, you’ll often find Silver plans with "Cost Sharing Reductions" that make your deductible almost disappear.
Georgia: Navigating the "Gatekeeper" Plans
Georgia's marketplace is robust, but many of the most affordable plans are HMOs (Health Maintenance Organizations). This means you need a "gatekeeper" (a primary care doctor) to give you a referral for everything. If you want the freedom to see a specialist in another city or state without a hall pass, you’ll likely need to look at a PPO alternative or a higher-tier Gold ACA plan.
ACA vs. Short-Term Medical: The PPO Freedom Angle
We hear it all the time: "I just want a plan that works everywhere."
Most "affordable" ACA plans in TX, FL, and GA are HMOs or EPOs. They work great if you stay in your county, but they can be a nightmare if you travel or move frequently. This is where Short-Term Medical (STM) shines.
STM plans often utilize large, nationwide PPO networks. For gig workers or self-employed pros who travel for work, having that nationwide access is a huge relief.
Wait! Is STM "Real" Insurance?
Yes, but it’s different. It’s designed for people who are healthy and want to protect themselves against catastrophic events (like a car accident or a sudden appendectomy) without paying the full "Major Medical" price tag. It doesn't cover maternity or mental health, but it does provide a safety net that follows you across state lines.
