If you live in the Carolinas or the rolling hills of Tennessee, you know that life moves at a slightly different pace. Whether you’re a 1099 contractor in Nashville, a self-employed creative in Asheville, or running a small business in Charleston, one thing remains universal: finding affordable health insurance that actually works for you can feel like a part-time job you never applied for.
The landscape of health insurance has shifted significantly in 2026. Many of the "extra help" programs we saw a few years ago have phased out, and we’re back to the original rules of the system. This can be stressful, especially when you're navigating the differences between ACA plans, Short-Term Medical, and supplemental coverage on your own.
At Real Health Quote, we help people exactly like you, families, gig workers, and small business owners, cut through the noise. Here are 10 things you need to know about finding health insurance in TN, NC, and SC right now.
1. Licensed Agent vs. Healthcare.gov Navigator: Why it Matters
When you start looking for coverage, you’ll likely run into two types of people: Licensed Health/Life Insurance Agents and Healthcare.gov Navigators (or CMS support staff). It is vital to understand the difference.
Navigators are government-funded facilitators. They are trained to help you fill out the paperwork and navigate the website, but they are legally barred from giving you actual advice. They can't tell you which plan is better for your specific doctors or recommend a strategy that saves you money on premiums. They are simply there to process the application.
A Licensed Agent, like Michael Peck, is an expert advocate. Michael doesn't just help with the paperwork; he provides personalized recommendations based on your health history, your budget, and your local networks in cities like Knoxville or Raleigh. An agent stays with you year-round, helping you navigate claims or changes in your life. Best of all? It costs you exactly the same as doing it yourself.

2. Filter #1: Your Tax History Drives Your Discount
One of the most important things we use to narrow down your options is what we call "The Big Three Filters." The first filter is your Tax History.
If you are looking for an ACA (Affordable Care Act) plan, often referred to as Marketplace coverage, the government uses your tax filings to determine if you qualify for a subsidy. For the self-employed, this can be tricky. Your "Adjusted Gross Income" is the magic number. If you have a clean tax history and your income falls within certain ranges, you could see your monthly premiums drop significantly, sometimes all the way to $0.
However, if you are a 1099 worker with a complicated tax situation or if you don't want the IRS looking into your healthcare choices, ACA might not be the primary path for you.

3. Filter #2: Navigating Pre-existing Conditions
The second filter is your health history. ACA plans are "guaranteed issue," meaning they cannot deny you for pre-existing conditions. If you have an ongoing chronic illness, are pregnant, or have a surgery scheduled, an ACA plan is almost always your best bet.
On the other hand, Short-Term Medical (STM) plans, which are widely available in Tennessee, North Carolina, and South Carolina, do not cover pre-existing conditions. These plans are designed for people who are relatively healthy and want to save money. If you’re in good health, STM can offer a massive discount compared to full-price ACA plans.
4. Filter #3: The "Subsidy Cliff" is Back
In 2026, the "Subsidy Cliff" is a reality again. During the last few years, enhanced credits made ACA plans affordable even for high-earners. Those enhancements have expired.
Today, if your household income exceeds 400% of the Federal Poverty Level (roughly $64k for an individual or $132k for a family of four), you hit the cliff. This means your tax credit drops to zero, and you are responsible for the full, "sticker price" premium of an ACA plan. For many families in the South, this can mean paying $1,500 or more per month for a plan with a high deductible.
This is where having an agent is crucial. We can help you determine if you're about to fall off that cliff and what your alternatives are.
5. Short-Term Medical: Not Just for the "Short Term"
There is a common myth that Short-Term Medical (STM) is only for people between jobs for 30 days. In TN, NC, and SC, that simply isn't true. For many of our clients, STM acts as a high-quality "bridge" to long-term coverage.
While STM is not Major Medical, it is a powerful tool for those who don't qualify for subsidies. It provides a way to get coverage quickly, often starting the very next day, without waiting for an Open Enrollment period.
If you’re worried about the cost of Marketplace plans, call Rachel at 512-850-6604 to see if a PPO bridge plan is right for you.

6. The Freedom of Nationwide PPO Networks
One of the biggest frustrations with ACA plans in many Southern states is the network. Most Marketplace plans are HMOs (Health Maintenance Organizations), which require you to stay within a very specific local geographic area and often require referrals to see a specialist.
If you travel for work or live near state lines, like working in Charlotte, NC but living in Fort Mill, SC, an HMO can be a nightmare. Short-Term Medical plans, however, often utilize massive nationwide PPO (Preferred Provider Organization) networks. This gives you the freedom to see doctors across the country without needing a "permission slip" from a primary care physician.
7. Building Your Safety Net with Supplemental Coverage
Health insurance isn't just about your monthly premium; it's about what you pay when things actually go wrong. We often recommend a "layered" approach to coverage.
- Accident Insurance: This pays you a lump sum if you get hurt in an accident (think ER visits or broken bones).
- Hospital Indemnity: This pays you a fixed daily cash benefit for every day you are in the hospital.
By adding these low-cost riders, you can effectively "buy down" your deductible, ensuring that an unexpected trip to a hospital in Nashville doesn't break your bank account.
8. Critical Care and Life Insurance
When you're self-employed, you are the engine of your business. If you get a major diagnosis like cancer or a heart attack, the medical bills are only half the battle, the other half is the lost income.
Critical Care coverage provides a cash cushion during these times. Additionally, we always look at Term Life Insurance. Many people don't realize they can bundle these protections into their health insurance strategy for a very small additional cost. It’s about peace of mind for your family.
9. Don’t Forget Dental and Vision
While most people focus on the big medical stuff, your teeth and eyes need love too. ACA plans for adults rarely include comprehensive dental or vision. We offer standalone or bundled Dental and Vision options that provide immediate value, covering your cleanings and exams so you aren't paying out of pocket for the basics.

10. You Can Enroll Any Time of Year
Many people think they can only get insurance during the "Open Enrollment" window at the end of the year. While that’s true for ACA plans (unless you have a "Life Event" like moving or losing a job), other options are available 365 days a year.
Whether it’s May or December, we can help you find a plan that fits. You don't have to go without coverage just because you missed a government deadline.
